Project Planned Activities

Community Engagement:

  • Conduct community meetings, workshops, and health fairs to disseminate information about integrated health services.
  • Collaborate with local leaders to promote health initiatives.

Capacity Building:

  • Train community health workers (CHWs) to deliver integrated health services.
  • Provide technical support to local health facilities to improve service delivery.

Service Delivery:

  • Set up mobile clinics and outreach programs to provide services directly in underserved areas.
  • Facilitate referrals and linkages to specialized care where needed.

Monitoring and Evaluation (M&E):

Conduct regular reviews to ensure project goals are being met and adapt strategies as needed.

Establish systems to track service utilization rates, patient satisfaction, and health outcomes.

Target Population

The primary beneficiaries include:

  • Vulnerable populations such as women, children, and people living with chronic illnesses.
  • Marginalized communities with limited access to healthcare services.
  • Community health workers and local healthcare providers.
